Please find below a list of forms you may need during the season. (updated June 15, 2011 by kg)
For Referees:

Before the season:
1. Rugby Canada Registration form – we need you to register with Rugby Canada every year if you want to referee senior, junior or mini games in Alberta or Canada.
2. ERURS Off-Season Travel Support application – this is currently suspended.
3. Goal Setting Planning form – for referees to prepare pre-season goals.
During the season:
1. RAMOASendingOffReport – Complete this report as the Referee if you have given any Red Cards. You must submit the completed report to ERU Director of Discipline within 48h of your match.
2. RAMOAAssistantRefereeSendingOffReport – Complete this form as the Assistant Referee (AR) if your information resulted in a Red Card or Send-Off. You must also send your completed report to the ERU Director of Discipline within 48h of the match.
3. RAMOAtravelclaimform (travel in Alberta) – When you travel throughout Alberta to officiate matches, RAMOA has flat rates for each trip. If you have traveled to Edson, Red Deer, Cold Lake, Calgary, Fort McMurray, Grande Prairie, and even to Edmonton from any of these places to officiate a Rugby Alberta match, complete this form and send to the RAMOA Treasurer before October 31 of this year.
4. Arbiter Tip Sheet
After the season:
1. 2010 ERURS Claim – done through RAMOA and Arbiter. So, this form is now obsolete.
2. Feedback for Season form
For Groups that Need Referees:
